Alliance 2020 offers a simple, easy, and secure solution for DOT Physical Exam Scheduling. Clinics in the Greater Seattle area that currently provide verified walk-in DOT physical exams are listed below. You will be able to choose the one closest to you during the scheduling process.
Once you have completed the scheduling form, Alliance 2020 is notified. The order for the physical exam is placed through eScreen and the ePassport will be emailed directly to you within 24 hours. The ePassport must be taken with you to the clinic.

Once the physical exam is completed, the results are sent to Alliance 2020. The credentialing coordinator will be in touch with you or the company during this process.
